Terms and Conditions - Concordia Health One Month Free Fusion Membership

  1. Customers have to be registered with Concordia Health to qualify for the free month membership with Fusion Lifestyle
  2. Upon signing the contact consent form, the Concordia Health patient is authorising Concordia Health to pass the patient's contact details to Fusion Lifestyle. Fusion Lifestyle will then directly contact the Concordia Health patient to explain more about the offer
  3. The offer is valid for 30 days from the point of the customer signing the contact consent form
  4. Should the Concordia Health patient choose to take up the free offer, they will be required to register as a member with Fusion Lifestyle. To register with Fusion Lifestyle, the Concordia Health patient will be required to submit their Direct Debit details. NB: This does not mean that any financial commitment has been made. The first month of the membership will be free of charge and the membership fee is waivered
  5. After 3 weeks of the free one month membership, a member of Fusion Lifestyle will contact the Concordia Health patient to confirm if they want to continue the membership into a second month. If the Concordia Patient wishes to continue, they will be required to pay a membership fee (to be agreed with Fusion Lifestyle) from the second month onwards. The Direct Debit will take effect from the start of the second month. No Direct Debit will be transferred during the first month of membership
  6. If the Concordia Health patient does not wish to continue the membership, they will need to sign a membership cancellation form, at which stage the Direct Debit will be cancelled and no fee will have been transferred to Fusion Lifestyle
  7. In the event that Fusion Lifestyle is unsuccessful in contacting the Concordia Health patient, the membership will continue into the second month and a membership fee will be transferred from the Concordia Health patient to Fusion Lifestyle by Direct Debit
